LICENSED PRACTICAL NURSING JOB DESCRIPTION QUALIFICATIONS Graduate of a state school of practical (vocational) nursing and currently licensed in Michigan.
Experience in Nursing Services is preferred, best in homecare setting.
Pass agency testing for medication procedure, competency, & universal standard precautions.
Past without incident the OTIS, ICHAT Fingerprinting background checks.
Job details:
The nursing staff directs all patient care as required by Michigan state & Federal Minimum Standards.
The LPN shares information to the multidisciplinary team with consideration of adult & geriatric clients.
The LPN participates in the quality control of service delivery and reports to supervising RN any issues needing attentions.
Demonstrated ability to manage all aspects of Home Health Care including but not limited to:
Private Duty Nursing; Chore Services; in-home-respite; personal care; home-maker; Financial Management.
Referral for therapy as needed.
Responsibilities:
Assure patient charting is complete and in order.
Report to supervisor findings as needed.
Assure polices are in place to provide services with-in contract guidelines, findings reported to Administrator.
Participate in and perform in-service training as needed with Administration and Home Health Care associates.
Communicate well with all other staff relating to patient care and wellbeing.
Do in-home patient assessments as defined by Administration.
Perform in home supervisory evaluations of Home Health Care associates and other staff as assigned by supervisor.
Complete clinical and progressive notes as needed, assure good communication.
Direct services in accordance with physician orders and care plans.
Counsel patients, family and home care associates in meeting nursing and all related services to improve patient's wellbeing.
Coordinate care with Supervising RN, and Physician, Physical Therapists, Speech Pathologist, Occupational Therapist, and Medical Social Worker as instructed by Supervisor to assure patients needs are being met.
Perform in-home services with AT RISK clients in, an emergency situation.
Perform task as agreed and assigned by Administration.
WORK ENVIRONMENT Work will be performed in the Office, at LPN Home Office and in the patients' residents.
Pay is by hour; 1 hour = 1 visit including reasonable travel time and charting.
Additional mileage as contracted.
Office visit time at administrative rate, extra charting time must be approved.
LPN will be expected to follow all policies and procedures.
The nursing staff will always promote the agency in a positive manor and will follow dress code appearing neat, clean and professional.
Equipment and supplies will be provided by the Agency as need or requested.
SUPERVISION Reports to the Supervising Registered Nurses who also evaluates the LPN workload.
Other in-home staff will be ask to assist in evaluation.
Once a year the in-home care associates will fill out an evaluation form that will be used in supervision and employee performance evaluations.
Top Salary hourly rates, negotiated at time of interview.
Benifits negotiated after 90 days of postive review! Time-keeping procedure must be used to report time for pay and will be used in study for development.
